Von Willebrand DNA is the gold standard and is available for the following breeds: Bernese Mt. Dogs, Shelties, Pem. REQUIREMENTS, STIPULATIONS, PAYMENTS AND REFUNDS
· PAPERWORK: To provide these low prices all paperwork must be completed prior to the clinic. This form and a copy of your animal’s registration papers are required. You may fax them to the above number or mail them to the above address (ATTN: JUDY). All paperwork must be received by July 3rd, 2009 to hold your reservation.
· OFA: OFA Cardiac is exam only. Fees for OFA Thyroid are included in the published price. All other OFA fees are separate and are the responsibility of the owner. Bring a credit card or a check for these fees.
· MICROCHIPS: Microchip price includes chip and registration. There is no appointment needed for microchip implants during clinic hours.
· RADIOGRAPHS: Radiographs are digital and done without anesthesia/sedation. Do not expect perfect positioning. If the radiographs cannot be taken in a satisfactory manner without anesthesia/sedation you will have the option of making an appointment at regular hospital hours and paying the additional cost of anesthesia/sedation or you may elect a refund of your money to be mailed to you by check. All radiographs are evaluated by a technician, not a veterinarian. A copy of the radiographs will be automatically sent to OFA (OFA requires that we maintain a chain of custody and directly send them the radiographs) or you may take them for personal use. You may purchase additional radiograph copies at $15 per animal regardless of the number of radiographs of the animal. All radiographs are digital and are burned onto a Compact Disc. They can be viewed and manipulated on any computer.
· PAYMENT & RESERVATION: Payment is required to secure your reservation by July 3rd, 2009. Checks should be made payable to Arlington Animal Hospital.
· CANCELLATIONS, RESCHEDULING & REFUNDS: Prior to July 3rd, 2009 you may reschedule or cancel your appointment without any charge. After July 3rd, 2009 any unpaid appointments will be cancelled and no refunds will be issued for cancelled appointments. After July 3rd, 2009 you may reschedule your appointment to any available appointment without charge.
